1. This registration form is not for appointment booking or any emergency requests. For appointments / emergency, please call 044 4200 2288 or 044 2249 2288. 2. The Good Neighbours Privilege Card can be availed only if your residential / office address is in close proximity to Chennai. 3. This is a registration of interest form for the Good Neighbours Privilege Card and not a registration form. One of our executives will be in touch with you to take you through the registration process upon receiving your request. 4. Once the registration process is complete, our executive will contact you over the phone / email / SMS, so you can collect the Good Neighbours Privilege Card, which will be available at the Helpdesk at MIOT International, 4/112, Mount Poonamallee Road, Manapakkam, Chennai - 600 089. 5. The primary card holder’s government-authorised address proof needs to be shown (recent address proof in the last 6 months) at the counter before collecting the Good Neighbours Privilege Card. 6. The privileges of this card can be availed only for 1 primary member + 3 members of the same family. 7. Benefits of this card cannot be clubbed with any Government Schemes / Insurance Policies / Corporate Tie-ups. It is valid only for payments by Cash / Credit Card / Debit Card / Net Transfer. 8. To avail services and offers designated to you, please produce this card at the Registration / Billing / Admission Counter at MIOT International. 9. Offers / Discounts in the Good Neighbours Privilege Card are not applicable for Medicines / Implants / Blood Products. 10. This Card / Copy of the Card should be produced before Raising Bill / During Admission to avail the benefits. 11. MIOT International does not offer the following services: Family Medicine, Geriatrics, Psychiatry, Medical Genetics, Neonatology, Paediatric Gastroenterology, Unplanned LSCS High Risk Pregnancies in Obstetrics & Gynaecology and Treatment for Burns. 12. The Good Neighbours Privilege Card is non-transferrable or assignable to any other individual apart from the card holders’ family members. 13. MIOT International is not responsible for loss or damage to your Good Neighbours Privilege Card. 14. If your card is lost or damaged, please report it to us immediately. 15. MIOT International may, at its sole discretion, reject the Good Neighbours Privilege Card if there is any suspected fraudulence or misuse of the Card. 16. MIOT International reserves the right to update or modify the privileges, and Terms & Conditions of the Good Neighbours Privilege Card at its sole discretion with or without any prior notice. 17. MIOT International is not responsible if there is a delay in confirming your registration. We try our best to respond to your request on time. MIOT International is not liable if your online request does not get delivered to us, as it may be due to technical problems such as network connectivity, spam regulations or any other issues faced at the time of form submission. 19. When you send us the registration of interest form, MIOT International will not share your information with any third party service provider; however, we might use your information to contact you in future to deliver any healthcare benefits in the interest of helping and serving you better. 20.
